Membership Details and Terms
- Six-month initial commitment with automatic monthly payment
- One membership per patient
- Memberships and treatment bank funds may not be transferred or shared
- Treatment bank funds have no cash value and may not be redeemed for cash
- Member pricing may not be combined with promotional pricing
- Membership benefits remain active only while the account is current
- Cancellation requires 30 days’ written notice after completion of the initial term
- Unused treatment bank funds roll over while the membership remains active
- Prescription medications, GLP-1 medication costs, already-discounted packages, and promotional event offers are not eligible for treatment bank use
- Additional terms are contained in the signed Membership Agreement
